Howdy. I know this has been done on a standard 4LO wheel.
Any Pics from anyone who's had this done. If so who did it for you and what bits you used.

Another piece of information you may find useful is:
the bracket to mount the rear brake master cylinder, on Nuttynick's hybrid and many others, is available from me.
it is listed on the 350lc page on my website - www.linksengineering.co.uk
Nigel at NK did the same rear disc conversion for me on the now deceased pink lc and it worked very well.

looks a simple enough thing to do if you've got access to machining
standard 250 sprocket mounts into an alloy mount h/t bolts to the disc
mounts then the caliper mounts
think i would try to upsize the 12mm spindle but .....

Is this conversion best done on a 250 rear wheel or a 350?
These are the same size as i remember just the cush drive different?

that's pretty much the jist of the conversion Nick and me had done, however changing the bearings allowed the use of a 15mm spindle and NK designed the sprocket mount to include a cushdrive.

did you change the bearings?? and any info on the chain side how it bolts on and where to get one.... also spacers??? the caliper mount on the rear tom sells i can understand ,
but its the sprocket mount that interests me as i also want to do one...

Its obvious now to use a front wheel as they same size as the rear for the tyre!
